APCM is a single alternative that can be used in place of a number of current care management programs such as chronic care management, principal care management, transitional care management.

APCM stratifies patient population into three groups based on health risk and offers different levels of payments for them to encourage matching of care management intensity to health risk.

Not timed

APCM requires the availability of several services for enrolled patients but is not a timed code – there is no requirement for the services to exceed specific time thresholds (e.g., 20 mins for chronic care management) to be able to bill for it

Predictable reimbursements

APCM offers a fee for service payment of $15 per month for Level 1 patients, $50 per month for Level 2 patients; and $100 per month for Level 3 patients (national rates – actual local rates will vary slightly from these numbers).
